The same note format every time

Agreement to proceed, recorded
Each assessment opens with a spoken introduction in the patient's language, and the time the patient agreed to continue is recorded in the note. This is agreement to answer questions - informed consent for treatment still needs a conversation, ideally through an interpreter.
